Condition of pet on Arrival

(a)  All pet Shall be vaccinated for DHLPP, and these vaccinations MUST be up to date (Any vaccinations must be current to within 2 week after pet departing the centre and must be administered at least 10 days prior to arrival at the Centre) and a certificate of which must be produced to the Centre on arrival at the latest. The Centre may refuse to take any pet without production of this certificate. A vaccination for bordatella (Kennel Cough) is also recommended.

(b)  All pet shall be examined by the Centre’s staff or representatives to ensure pet is free from ecto parasites (eg: ticks and fleas) upon arrival then application of preventive medications (Frontline spot on) MUST be applied on the day the pet check(s) in and shall be witnessed or applied by the Centre’s staff or representatives. Subject to clause 1 of terms and conditions, the Centre at the applicant own cost will treat any pet, which requires medication or veterinarian.

(c)  The Centre reserves the right to refuse any pet/s or to terminate this application at any time before or during its term where the centre, in its sole discretion, determines that applicant’s be poses a danger to the health and safety of the Centre’s staff and guests.

(d)  Subject to clause 3(c) of this application form, the Centre shall have absolute right to place the pet in a kennel, with all charges as mentioned in the schedule of payment shall be incurred and to be charged to the applicant and the applicant shall agree to pay for such charges.

宠物的入住条件

 a)  所有入住的宠物必须完成DHLPP疫苗注射，并且疫苗必须保持处于有效状态（任何疫苗接种必须是从宠物离开中心后的2周内有效，并且必须等待至少10天后才到达中心）且其证书必须在抵达中心前为最新日期。本中心有权拒绝任何不携带此证书或预期的宠物。本中心也建议注射Bordetella（博德氏菌）疫苗。

 b) 所有的宠物必须由中心的工作人员或代表进行审查，以确保宠物不受外生寄生虫（如：虱子和跳蚤）的侵袭，然后于当天必须由中心的工作人员或代表适当地应用预防性药物（FRONTLINE SPOT ON）。鉴于第1条款，任何药物与治疗费用皆由顾客本人负责。

 c)   本中心有权拒绝任何宠物或任何已经光临本中心的顾客的入住申请，必须根据本中心的审核标准进行审核以确保所有的宠物和顾客的健康与安全。

 d) 根据C条款，该中心将拥有绝对的权利把宠物安置于适合的房间，与之所有费用中提到的付款也经已获得申请人之同意以支付这些相关费用。
